Online Re-Registration

15 STEPS

1. Visit https://ma-leominster.myfollett.com/

Click on Initiate

2. Select your child - you can only select one child at a time

3. Click Next

4. On this page, you can:

Update and/or add phone numbers
Agree/Disagree to the various consent areas

5. Once you're done, click Next

6. On this page, parent/guardian information can be updated.

To make a change, click on the guardian's profile you wish to edit

7. Any information you want updated on this page, needs to be noted in the "Contact Notes" box

You can type in the box - for example you could write: Please update my email address to parent3@gmail.com

8. When you are done typing your requests in, scroll down and click "OK"

9. When you are done updating all profiles, click "Next"

10. ALL fields on this page need to be reviewed and updated - this is the medical field page

11. Your name should automatically appear in this box, but if not, please add it.

12. For the date, you must click on the calendar and select the current date.

13. Click "Next"

14. Click Submit

15. That's it. You're done.
